Weeks 1-6 5/1 mg/ml 0.5ml Vial

Inject 0.05 ml (5 units) subcutaneously weekly for 2 weeks.  Then Inject 0.1 ml (10 units) subcutaneously weekly for 4 weeks. Do not increase dose if severe side effects are present.

Weeks 7-12 5/1 mg/ml 1ml Vial

Inject 0.15 ml (15 units) subcutaneously weekly for 4 weeks. Then Inject 0.20 ml (20 units) subcutaneously weekly for 2 weeks

Week 13-18 5/1 mg/ml 2ml Vial 

Inject 0.20 ml (20 units) subcutaneously weekly for 2 weeks. Then Inject 0.35 ml (35 units) subcutaneously weekly for 4 weeks

Week 19 Onward 5/1 mg/ml 2ml Vial 

Inject 0.5 ml (50 units) subcutaneously weekly. May decrease by 0.05 ML (5 units) if side effects persist

The Process:

1)Schedule a tentative 15 min telehealth appointment with one of our providers by visiting our website and clicking “Schedule Appointment”

2)You will receive our new patient/medical history form within 24 hours along with a payment link to enroll in the program. Both will need to be completed before your telehealth visit is confirmed.

3) One of our providers will review and accept your appointment, and send a confirmation email of scheduled date and time. Please be present and on time for your scheduled telehealth appointment.

Tirzepatide Dosing Schedule

RX #1: Tirzepatide/Glycine/B12 10mg/5mg/500mcg/ml – Inject 25 units subcutaneously weekly for 4 weeks, then increase
to 50 units weekly for 2 weeks (1 x 2ml vial)

RX #2: Tirzepatide/Glycine/B12 10mg/5mg/500mcg/ml – Inject 50 units subcutaneously weekly for 2 weeks, then increase
to 75 units weekly for 4 weeks (2 x 2ml vials)

RX #3: Tirzepatide/Glycine/B12 10mg/5mg/500mcg/ml – Inject 100 units subcutaneously weekly for 4 weeks (2 x 2ml vials)

RX #4: Tirzepatide/Glycine/B12 10mg/5mg/500mcg/ml – Inject 125 units subcutaneously weekly for 4 weeks, then
increase to 150 units weekly thereafter (3 x 2ml vials)

RX #5: Tirzepatide/Glycine/B12 10mg/5mg/500mcg/ml – Inject 150 units subcutaneously weekly for 4 weeks (3 x 2ml
vials)
